Should it be:

"He has earned Semester Academic Honors (which require a 3.5 grade point average in a minimum of twelve credits) in two semesters, and Dean’s List Honors (which require a 3.75 grade point average in a minimum of twelve credits) in one semester."

or

"He has earned Semester Academic Honors (which requires a 3.5 grade point average in a minimum of twelve credits) in two semesters, and Dean’s List Honors (which requires a 3.75 grade point average in a minimum of twelve credits) in one semester."

I think it's the latter... "he has earned x, which requires..." but I thought I'd throw it out there in case I'm smoking crack.

"Semester Academic Honors" is a singular subject. Therefore, "it requires."
